At Hope Bridge Home (HBH) Residential Treatment Center, we strive to provide a safe, nurturing, and healing environment for children who have faced trauma and adversity. We are committed to serving children who require specialized care and support, ensuring they have the opportunity to heal, grow, and achieve their full potential.
Eligibility Requirements
Who Can Apply:

Children Ages 6-12
We accept boys and girls within this age range who require residential therapeutic care due to trauma, neglect, abuse, abandonment, or other adverse circumstances.

Children in Need of Specialized Services
We serve children with emotional, behavioral, and mental health needs, as well as those who require trauma-informed care.

Referrals
We accept referrals from state agencies, legal guardians, foster care systems, and licensed healthcare professionals.
Admission Criteria:
- The child must require therapeutic and residential care based on assessments from licensed mental health professionals.
- The child must not pose a significant danger to themselves or others, allowing for their safe inclusion in a communal residential setting.
- We serve both emergency and non-emergency admissions, with specific considerations for children entering in urgent care situations.

Initial Contact:
Families, caseworkers, or referring professionals can reach out to our Admissions Office via phone or email. Our admissions team will provide an overview of our program and determine if Hope Bridge Home is the right fit for the child.

Application:
- Once contact is made, we will send an Admission Application Form to be completed by the parent, guardian, or referring professional.
- The application requires information about the child's medical, psychological, and social history, as well as details about previous care or interventions received.

- A formal referral from a licensed professional (if applicable).
- Proof of guardianship (if not referred by state agencies).
- Any previous individualized treatment or service plans.
Assessment:
- Our professional service team will review the application and assess whether the child’s needs align with our services.
- A face-to-face or virtual evaluation with the child may be required to better understand their unique needs and ensure appropriate placement.
Acceptance:
- If the assessment team determines that Hope Bridge Home is the right placement, we will notify the family or referring professional and provide an acceptance letter.
- At this stage, the admissions team will schedule an intake appointment and assist in transitioning the child to our care.
